1. Malpractice Liability Insurance
  2. Term Life Insurance in the amount of one times annual base salary up to a maximum of $250,000, subject to insurability at standard premiums, provided by CCHA master plan
  3. Health and Medical Care employee-only coverage insurance, including dental insurance, subject to insurability, provided by CCHA master plan.  Premiums, which are equivalent to the full-time employee current premium rates, are to be paid by Doctor and will then be reimbursed through the bi-weekly payroll.
  4. Upon documentation of completion of CME each year, reimburse up to $500 annually, per CCHA policies covering CME.
  5. Annual non-cumulative leave of 15 business days for vacation, sick time, and holiday.  Eligible for FMLA in compliance with the Family and Medical Leave Act of 1994.

  11. BLS, ACLS, PALS, NRP training will be provided through the hospital education department.
  12. Additionally for Emergency Medicine Residents ATLS will be provided by an outside agency and will be paid for by CCMH
  13. Two sets of scrubs and 2 physician monogramed coats will be provided.
  14. For Emergency Medicine: PGY-3 will receive a stipend of $2,500 to attend ACOEP Conference to present their research project.
  15. For Family Medicine: PGY-3 will receive a stipend of $2,500 to attend ACOFP Conference.
  16. American Osteopathic Association or American Medical Association dues will be paid.
  17. For Family Medicine: American College of Family Physicians or American Academy of Family Physicians dues will be paid.
  18. For Emergency Medicine: Emergency Medicine Residents’ Association or ____________ dues will be paid
  19. Oklahoma Osteopathic Association dues will be paid.
  20. For Emergency Medicine: Rosen’s Emergency Medicine will be supplied.
  21. Resident Inservice Exams will be paid
  22. Oklahoma Bureau of Narcotics and Dangerous Drugs will be paid throughout residency.
  23. Drug Enforcement Administration will be paid throughout residency.
